Skilled Nursing Care
Our nurses help make the transition to the home easier by working closely with patients, physicians and family members. Nurses also develop individual care plans to treat acute or chronic health problems under physician orders. Our services include medical assessment, wound care, pain management, patient education, medication management, IV therapy, diabetic care, ostomy care, cardiac care and coordination of care by case manager., coordination for oxygen and respiratory therapy. Medical equipment and supplies are ordered through our network of suppliers or the supplier of your choice.
Home Health Aide Services
Home health aides help patients with activities for their basic everyday needs such as: bathing, grooming, dressing, eating, walking or simply getting out of bed. Their presence has a positive impact on the quality of life for patients and their families.
Physical Therapy
Physical therapists help patients regain and improve mobility after hospitalization or any procedure. They provide safety assessments and teach balance and therapeutic exercises.
Occupational Therapy
Using specially developed exercises, occupational therapists assist patients with daily living skills including bathing, dressing and preparing meals.
Speech Language Therapy
Our therapists help patients who have difficulty speaking learn to make sounds, understand speech and use other communication skills. For patients who have difficulty swallowing due to injury or health conditions, speech therapists employ reflex and sensory-stimulation treatment.
Medical Social Worker
Social workers assess social functioning of patients and families and intervene as necessary. Interventions may include connecting patients and families to necessary resources and supports in the community; supportive counseling, or grief counseling; or helping a patient to expand and strengthen their network of social supports
